Questionnaire configuration
There are multiple scenarios in which information about a party (borrower, co-borrower, guarantor, and so on) or collateral must be gathered as a part of the loan or lease origination process. Q2 Origination supports an all-new questionnaire framework that enables banks and FIs to configure their own questionnaire and specify the answer format to gather information. Each questionnaire can have multiple sections, and each section can have multiple questions. Each question can have possible answers. Each answer can be associated with a score. A question can be associated with a display condition if an answer further has a dependent question. The total score of a questionnaire can drive the next steps in the workflow.
These questionnaires can be inserted anywhere in the origination workflow to gather data. Banks and FIs can effectively use this questionnaire framework to gather information related to customer information programs, applicant demographic data for government monitoring information (GMI), customer profiling questions, and more. All the information gathered as part of this questionnaire can be easily extracted for review by compliance officers.
For example, based on the responses to an environmental questionnaire, the credit officer marks the collateral as 'Environmental Risk -Yes/No.' Once the property is identified as an environmental risk, a phase 1 environmental site assessment is initiated. This assessment is usually done by a third-party vendor or a qualified bank representative.
Configure a questionnaire
Read this section to understand how to structure and create a questionnaire.
A questionnaire can be associated with an application, a party, or collateral.
A questionnaire can be configured to have multiple sections.
Each section can have multiple questions.
Each question can have possible answers.
Each answer can be associated with a score.
A question can be associated with a display condition if an answer further has a dependent question.

Steps
To configure a questionnaire:
Log in to your Salesforce account.
Go to (App Launcher) > Launchpad.
Select Questionnaire Configuration.
The Questionnaire Definition page is displayed.
Select .
Specify the Questionnaire Definition Name and Object Type.
The supported Object Types are:
Application
Party
Application Collateral
Select Save or Cancel if you do not want to create the questionnaire.
On the Questionnaire Definition page:
Select the created Questionnaire Definition Name and then select Add Section.
You can expand the section to update the section name.
Note:If the questionnaire is published or deactivated, the Add Section option is not displayed.
To add questions to the sections, select Add Question .
Specify the required field values as described in the following table, and then select Save:
Note:The fields are displayed based on the selected Answer Type .
Field Name Action and Information Section Name You can group the questions into different sections and use this field to give relevant names to those sections.
For example, you can create a section with the name Personal Identification to display all the questions related to personal identification in it.
Question Description Use this field to give a relevant description of the question.
For example, for a gender-related question, the description can be

.Answer Type Use this field to define the type of answer that a user gives. You can associate an answer type with every question. An answer can be of the following types:
Check Box
Picklist
Text Area
Multiselect Picklist
Enable Score Select this checkbox to capture a score for the answer(s) recorded for a question.
Is Required Select this checkbox to make a question mandatory. Value Specify the potential answers or values to the question. These values depend on the Answer Type selected in the previous step.
Let us say that you are asking a user to specify their gender, and the answer type is a picklist. You can specify the values as Male, Female, Trans, and Do Not Want To Disclose.
Score An answer value can be associated with a score or weightage. The scores can be used in decision-making.
Let us say that you are creating a questionnaire to find out the female population in a locality to understand the scope of business related to female products in that area. So, you can associate a high score with the value of females and also the female users of certain products. You can then aggregate the scores to understand whether the locality would be a profitable market for your products.
Add Display Condition
This option appears only after defining a section with at least one question. Use this option to define the conditions related to questions that, if satisfied, display associated questions.
Use Section ID and Question ID to derive the question on which a condition is to be set.
Define the condition with the help of the Operator.
Specify the Answer Value that, when met, would satisfy the defined condition.
To cancel the question definition, select Cancel.
To delete a question definition section, select (Delete Question Definition).
To delete a section, select Delete Section.
To view the created questionnaire definition, select Preview.
To save the questionnaire definition and publish it later, select Save as Draft.
To publish the created questionnaire definition, select Publish.
The status of the Questionnaire Definition is updated to Published.
Questionnaire Definition Name -
Name of the questionnaire that is configured for an object.
Object Type -
The object on which the questionnaire is configured. It can be Application, Party, or Application Collateral.
Status -
Published - When a questionnaire is published
Deactivated - When a questionnaire is deactivated
Draft - When a questionnaire is configured but is not published
Action
Edit Row - This option is to edit the details like Questionnaire Definition Name and Object Type. This option is available only when the questionnaire is in Draft status.
Publish - This option is to publish a questionnaire that is either in Draft or Deactivated status.
Flag For Deletion - This option is to delete a questionnaire when it is configured or in Draft status.
Deactivate the questionnaire - This option is to deactivate a published questionnaire.
Deactivate a questionnaire
You can deactivate a questionnaire that is published. A published questionnaire is listed with the Status as Published as highlighted in the following image:

Before you deactivate a questionnaire, ensure that the following prerequisite is met:
The questionnaire to be deactivated is published.
Steps
To deactivate a questionnaire:
From the list of Questionnaire Definitions, select the Action list for the questionnaire that you want to deactivate, select Deactivate the questionnaire and then select Save.
When deactivated, the same questionnaire is listed with the Status as Deactivated, as highlighted in the following image:
You can activate a deactivated questionnaire by selecting the Publish option from the Action list.
