Create questionnaire definition
There are multiple scenarios in which information about a borrower or collateral must be gathered as a part of the loan or lease origination process. Q2 Origination has come out with an all-new questionnaire framework that enables banks to configure their own questionnaire and specify the answer format to gather information. Each answer can be configured to be associated with a score. The total score of a questionnaire can drive the next steps in the workflow.
These questionnaires can be inserted anywhere in the origination workflow to gather data. Banks can effectively use this questionnaire framework to gather information related to customer information program, applicant demographic data for government monitoring information (GMI), customer profiling questions, and more. All the information gathered as a part of this questionnaire can be easily extracted for review by compliance officers.
For example, based on the responses to an environmental questionnaire, the credit officer marks the collateral as Environmental Risk -Yes/No. Once the property is identified as an environmental risk, a phase 1 environmental site assessment is initiated. This assessment is usually done by a third-party vendor or a qualified bank representative.
Create a Questionnaire
Read this section to understand how to structure and create a questionnaire.
A questionnaire can be associated with Application, Party, or Collateral.
A questionnaire can be configured to have multiple sections.
Each section can have multiple questions.
Each question can have possible answers.
Each answer can be associated with a score.
A question can be associated with a display condition if an answer further has a dependent question.

Steps
To configure a questionnaire, perform the following steps:
Log in to your Salesforce account.
Select (App Launcher).
Search for
Launchpad
, and then select it.Select Questionnaire Configuration.
The Questionnaire Definition page is displayed.
Select (Add New Questionnaire Definition).
Specify the required Questionnaire Definition Name and Object Type.
The supported Object Types are:
Application
Party
Application Collateral
Select Save.
On the Questionnaire Definition page,
Select the created Questionnaire Definition Name > Add Section.
You can expand the section to update the required section name.
To add questions to the sections, select Add Question.
Select Define Question .
The window appears to specify the required description and scores.
Specify the required field values as described in the following table, and then select Save:
Note:The fields are displayed based on the selected Answer Type.
Field Name Action and Information Section Name You can group the questions into different sections, and use this field to give relevant names to those sections.
For example, you can create a section with the name Personal Identification to display all the questions related to personal identification in it.
Question Description Use this field to give a relevant description for the question.
For example for a gender-related question, the description can be as Specify Your Gender.
Answer Type Use this field to define the type of answer that a user gives. You can associate an answer type with every question. An answer can be of the following types:
Check Box
Dropdown
Text Area
Multiselect list box
Enable Score Select this check box to enable the capturing of a score for the answers to a question.
Is required Select this check box to make a question mandatory. Value Specify the potential answers or values to the question. These values depend on the answer type selected in the previous step.
Let us say that you are asking a user to specify their gender, and the answer type is a picklist, then you can specify the values as Male, Female, Trans, Do Not Want To Disclose.
Score An answer value can be associated with a score or weightage. The scores can be used in decision-making.
Let us say that you are creating a questionnaire to find out the female population in a locality to understand the scope of business related to female products in that area. So, you can associate a high score to the value as female and also to the female users of certain products. You can then aggregate the scores to reach an understanding of whether the locality would be a beneficial market for your products.
Add Display Condition section
This section appears only after defining at least one section with at least one question. Use this section to define the conditions related to questions that if satisfied display associated questions.
Use the combination of the Section ID and the Question ID to derive the question on which a condition is to be set.
Define the condition with the help of the Operator.
Specify the Answer Value that when met would satisfy the defined condition.
To cancel the question definition, select Cancel.
To delete a questionnaire definition section, select Delete Section.
To view the created Questionnaire Definition, select Preview.
To publish the created Questionnaire Definition, select Publish.
The status of the Questionnaire Definition is updated to Published.
Deactivate a Questionnaire
You can deactivate a questionnaire that is published. A published questionnaire is depicted is with a power icon as highlighted in the following image:
Prerequisites
The questionnaire to be deactivated is published.
Steps
To deactivate a questionnaire, perform the following steps:
From the list of questionnaires, select the one that you want to deactivate by selecting the power button, and then select Save.
When deactivated, a questionnaire is listed with the published icon as highlighted in the following image:
Example
This example describes the creation of a questionnaire named, Environmental Inquiry, to determine whether a property is an environmental risk or not.
Based on the responses to this environmental questionnaire, the credit officer marks whether the property is an environmental risk.
Question 1: The answer type for this question is defined as a picklist so that the users can select from the listed options, which are Yes and No.
Each answer has a score attached to it. So, a Yes adds 1 to the total score of the questionnaire.
Question 2: Is displayed only when the display condition of Question 1 is satisfied, that is the response to it is Yes.
After the successful creation of the Environmental Inquiry questionnaire, it can be accessed from the Actions menu of any application as it was created on the Application object. When a user selects it from an application dashboard, the defined questions are displayed. And if the user selects Yes as a response to the first question, then the associated display condition is satisfied and the associated question is displayed as highlighted in the following image:
